The unauthorized release of information is a very serious matter. Your records are always kept confidential. This means that information regarding testing, diagnosis and treatment services provided at the clinic is privileged and will only be released to your treating physician upon request. [Note: There is a small charge for most record requests. Clinic staff will inform you of the cost at the time of request.] You may request your records be shared with others by submitting an Authorization for Release of Records form. However, records about a patients HIV status will not be released unless the release form specifically lists HIV information. Medical records are protected from release by law.
